This is caused when the actual fuel levels in the tanks are different to what the FMC has predicted you will be carrying a that point of the flight. Can be caused by many things (fuel leak, fuel dump, strong head winds) but i sem to remember (have slept a lot since then) that the last time i had this was caused by me forgetting to turn off the APU. Thsi also causes a 'Fuel Config' warning as the APU sucks from the left wing tank and this will be at a lower level.
What the FUEL DISAGREE-PROG 2 message is telling you is to look at page 2 of the progress page on the FMC to compare the totalizer and calculated fuel quantities. This way you can see at a glance where the problem is. Check the overhead panel to make sure the APU is not running (as Paul mentions it is a very common cause).
